Feedback and Complaints

If you have a concern or complaint about your current NDIS support or services by Proactive Behaviour Support, it’s important to talk about it. Fill out this complaint form below and we will respond to you within 2 business days. We will take reasonable steps to ensure that information provided in a complaint and during any complaints process is kept confidential and only disclosed if required by law or if the disclosure is otherwise appropriate in the circumstances.

A complaint may be made on an anonymous basis. You can make an anonymous complaint:

  1. Calling us on 0412867532 and stating that you wish to make an anonymous complaint (so we don’t ask you to identify yourself); or
  2. Filling out the Feedback and Complaints Form but not including your name and the other details in the table below as these may identify you and post it to 127 Perseverance Road, Vista SA 5091.

It is a guiding principle of the National Disability Insurance Scheme Act 2013 (NDIS Act) that people with disability have the same right as other members of Australian society to pursue any grievance.

If the participant wishes to give Proactive Behaviour Support feedback or is not happy with the provision of supports and wishes to make a complaint, the participant can talk to Amanda Bodycote on 0412 867 532 or email amanda@proactivebehavioursupport.com.au. We also value any feedback as an opportunity for learning and improvement of our service delivery.

If the participant is not satisfied or does not want to talk to this person, at any time, they can make a complaint to the NDIS Commission. Complaints to the NDIS Commission can be lodged:
